Web“Carer Champions recognise that carers sometimes simply just need a break. We give information out on local befriending services, we offer them a free carer assessment that is done by one of the Care Coordinators and they assess the needs of the carer, looking at how best we can support them. Webavailable for carers at any given time; a Carer Champion is a point of contact both internally for staff and carers and externally for professionals in order to ensure effective communication is happening across health care, social care and the third sector.
